HOME VISITOR RESPONSIBILITIES:
• Evaluating the effectiveness of the plan of care for the individual
• Assessing the participant’s health, welfare and unmet needs
• Developing back-up plans and goals with the client
• Communicating with SeniorsPlus Care Coordinators to assist clients in receiving appropriate, effective and efficient services to achieve the maximum amount of independence possible
• Positively impacting and improving the well-being and health care outcomes for disabled children, adults and older people who are participating in at-home, long-term care programs as prescribed.
EDUCATION and/or EXPERIENCE
• Bachelor's Degree in Social or Human Services or Health related required
• Familiarity with the long-term care system a plus, strong computer and organizational skills required
• Valid Driver's License a Must
